2/5/2023 EnchantaVaughn Approved 1. Behavior: Upon arrival, client was seen playing outside with his brother and sister. Client seemed to be in an active state and responding well to authority instructions. Client said he wanted to have one on one with his counselor and figure out some things. Client expressed that he didnât want to talk about in front of his mom because he didnât want to be judged for his mistakes. Client is still adjusting to the changes in the home and giving his mom space for self-care needs. Client shared positive and negative thoughts concerning his expectations and responsibilities. Client expresses that he misses having all the attention but understands that he must lead by example. Client explained his frustrations and why he has been more stressed than normal lately. Client wants to be able to balance everything and still be good. Client is very active and engaging with QMHP. Mom communicated client can take his break after all of his therapy assignments are complete or if he earns free time. Client appreciates getting the help that he needs and having someone he can talk to about everything. 2. Behavior: Client was not happy with the way his mom expressed his recent behaviors and became very silent in session. Client opened up about a lot of the insecurities he was starting to experience and why he has been closed off and distant some days. Client remained seated and was careful not to talk to loud, so he wouldnât disrupt others. Client demonstrated good manners to QMHP by being courteous and polite. Client engaged in conversation by asking and answering appropriate questions. Client gave good feedback, indicating he was listening and interested in the psycho educational activity. Client displayed good social skills throughout the session. Client was supported for his cooperation and wanting to address challenges in the community and at home. Client admits to taking his frustration out on others and not giving his mom the respect, she deserves. Client assures that he is learning and will prove it by improving his attitude and affirmations going forward. Client implemented responsible behavior and was given a thumbs up by mental health professional that he was doing better. 1. Intervention: QMHP applauded client for making the big decision to want to work on changing his behavior. QMHP discussed with him the importance of working hard to make sure that he continues to stay focused on his goals. QMHP challenged client to continue displaying a positive attitude while employing positive reinforcement when client is able to achieve his goals and objectives. QMHP provided praise when the client continued to display positive behaviors throughout the day, remaining on task and was very respectful to all adult figures. QMHP isolated client from distractions to decrease disruptive behaviors and to reduce excessive talking. QMHP guided client by asking him why he believed his family was important to him and what is the overall image that he wants to depict when it comes to his family. QMHP assisted client in creating goals for his family and understanding what role would be in achieving them. QMHP role played with client and demonstrated for positive behaviors and responses to increase positive interactions between him and the family. QMHP applauded the behavior of client and recognized how mother and he had been interacting positively. 2. Intervention: QMHP motivated client to respond to exercise and express his feelings appropriately. QMHP offered active listening skills, as well as, comfort in talking about personal experiences. QMHP provided client with encouragement to demonstrate the positive behaviors and expectations they had been talking about. QMHP listened as the client communicated his feelings with his dad. QMHP supported client in approaching his mother with his ideas. Once the conversation with the pair occurred, QMHP began a game with the pair that allowed them to mirror different emotions. Each member shared what they saw looking at the person and what they loved most about each other. They were to also discuss why they were important to each other and what the first goal was they would like to accomplish when bettering their relationship. QMHP engaged in conversation with client about ways to continue improving his behavior and interactions around his family and friends. QMHP processed with the client the feelings he shared and assisted him in sorting through them. QMHP encouraged the client to continue to work on understanding his feelings and making sure that he responds in a positive manner. 1. Response: The client responded in a constructive manner acknowledging the affirmative reinforcement that the QMHP provided. Client utilized relaxation strategies appropriately. The client showed his gratitude for being able to release physical tension stating that he felt relaxed. Client was able to cooperate and comply to rules and regulations due to positive reinforcement provided by the QMHP. Client implemented pro social skills successfully. Client was able to utilize coping skills to eliminate stressors and unwanted feelings. Client displayed self determination to stay focused and pay attention in class. Client utilized meditation and relaxation techniques. to decrease anger and frustration. Client reported a decrease in symptoms following the relaxation techniques. Client verbalized positive feelings about building relationships in the home, despite him feeling alone and unloved. Client is making progress with expressing his feelings and showing others respect and courtesy by talking with appropriate tone, initiating conversation with peers, and waiting for his turn. Client has presented himself in a responsible way and is making progress towards his goals and objectives. Response: The interventions were viewed as effective. Client communicated his likes and dislikes. Client recognized his triggers and was able to identify common interests and similarities him and other ADHD recipients share. Client communicated by nodding and exhibiting good eye contact. Client verbalized positive feelings about goals and objectives. Client communicated that he wants to work independently. Client displayed good participation in various settings and was responsive to all instructions. Client understands that he is expected to act civilized and treat others with respect. Client responds well to praise, support, and positive reinforcement provided by QMHP and is seen increasing self-esteem and confidence. Client said he was going to practice coping strategies and relaxation techniques in all aspects of his daily living routine in hopes that he will be a better son, brother, and friend. Client was able to cooperate and comply to rules and regulations due to positive peer influence. Client implemented pro social skills successfully. Client attends
